How does the doctor calculate a diet and why a diet?

If you read this page because in your mind you feel inadequate and obese even if this is not true, then it could be a problem of anorexia; so look for >> anorexia

The diet in medicine

Given that they are incorrect statements to base a diet only on a type of food, for example, talk about the diet of watermelon, banana, papaya etc.
What, however, is a serious diet and how should it be calculated?
Let's start with the stressed concept of overweight and obesity.

Definition of obesity

There are 3 definitions:
- a physiological one, for which it's said that a subject is obese if he has an increase in fat mass more than 25% if a man, more than 20% if a woman;
a psycho-social one, that is the belief that being "skinny" is beautiful and sexy;
- a medical one, for which the normal weight is the one that allows us to live better and longer (eg without back pain, without excessive load on an arthritic knee, etc.), far from cardiovasculometabolic complications.

What's the ideal weight?

There is a table of Quetelet or Body Mass Index (I.M.C.) which is calculated as follows:

weight / height in meters 2

The I.M.C. it must be <a 25 for the man and <a 20 for the woman; who is between 25-30 is overweight, who exceeds the value of 30 or even 40 is obese.

For example. the webmaster of this website weighs 95 kg and is 1,78 high therefore (95 / 1,782 ) = (95/ 3,1684) = 29,983 (weighed 95 !! .. for which he is almost overweight pushed ! But it is not meat that matters; it is the spirit that is strong!

Another classification of obesity causes it to be subdivided into

a) central or android or abdominal
b) peripheral, gynoid or hips;

Pathophysiology

It gets fat because there are hereditary factors, wrong eating behaviors that are acquired in childhood, mothers and overprotective fathers who swallow you, or because you have predisposition to diabetes and there is more insulinemia in the bloodstream, for endocrine disorders and ovary (eg women with hirsutism and fuzz on the face, hump, lunar facies etc.)

You must compare causes of obesity and hormonal imbalances  or obesity and mental illness

The diet recommended on our site

Let us always remember that it is your doctor who prescribes a diet. This scientific article is only a reading
In this website can find diets for all the most important diseases, for example the diet for the diabetic patient, and how to calculate the insulin  in the diabetic type 1, what foods you must choose in diabetes on the basis of the glycemic index. and how to develop a diabetes diet and calculate insulin units and food substitutions in the diabetic diet. We still talk about the diet for the patient with the dyslipidemia (diet for high cholesterol and triglycerides), or the water diet (The diet with the right water, mineral, minerals, laxatives, etc.) and if drinking a lot of water is always good (water poisoning)

Why then obesity?

Causes of obesity

Before starting a diet, you need to investigate if there are any problems at the base of your obesity, for example if you are becoming diabetic and have hyperinsulinemia, if your belly is increasing or your hips and your parents were diabetic, if you are having hair on the body, or if you're more tired lately and your feet are swollen, if you're urinating less. In these cases think of diabetes, think of hormonal diseases, think of heart diseases and talk to your doctor.
- Inheritance: 69% of the obese have an obese parent, 18% both;
- Environment: hyper-nutrition increases the mass of adipocytes (fat cells);
- Food behavior: ex. bulimia, hunger from ox, eat often, munch on sweet things and chocolate because you are depressed, snacks out of hours etc.
- Tendency to type 2 diabetes: between 25 and 50% of obese subjects tend to hyperinsulinemia and obesity and hypoglycemic crises, so it is always hungry
- Other endocrine disorders: thyroid (see Thyroid: introductory part The Thyroid: diseases associated with hyper and hypothyroidism) and the adrenal gland, ovarian disorders, excess of male hormones in women, so the weight is distributed as for man on belly!
- iatrogenic obesity: when the doctor prescribes a diet, and then the patient, after the diet, spontaneously resumes feeding before !!

What benefits with diet?

Balance: the assessment of circulatory, respiratory, joint, coxarthrosis, gonarthrosis, improve after diet therapy, the value of triglyceride and the LDL cholesterol is reduced, the same for value of uric acid.

The survey should allow the doctor to know the eating habits of the subject, then assess the initial calorie level, which is subtracted from the diet and is divided by nine, to obtain the value of weight reduction.

Eg. Who writes every day takes about 2000 Kcal, assuming that it's necessary a diet of 1000 Kcal/day to lose weight, then when will it be able to lose weight in a day?

>2000 - 1000/9 = 111.11 grams

in fact 9 Kcal is equivalent to the energy value that gives 1 gram of fat when it burns

However, this is not always the case, because slowly the body is rearranged, absorbs more, metabolizes better and the weight is reduced less. Even in the course of 5-9 years we recover the lost weight and even get fat! Fortunately, there are foods called functional foods that help you. These include soy lecithin and green tea products, as well as calcium based preparations that make the weight gain. It goes without saying that in the meantime you have to move and change your >>  lifestyle.

However, if a guy weighs 100 kg and must lose weight for 20 kg, ie reach 80 kg of weight, and initially takes 2200 Kcal in a day, another empirical formula is used: calculate the value in calories to be administered by multiplying 20 Kcal x weight to reduce, such as in the example:

2200 - (20 Kcal X 20 kg of weight to lose) = 1800 Kcal

From this concept we start to build the diet that must always be balanced, ie contain all the food principles, eg. 50-60% carbohydrates, 25-30% proteins and the rest in lipids, better if polyunsaturated oils (eg sunflower oil, olive oil) as they improve the catabolism of lipids (lipids). It is also advisable for the patient to take 500 mg / day of calcium, especially if he is losing weight because he could have osteoporosis and overweight pain, and he must take a polyvitamin complex.

Diabetes is also controlled by diet as well as by drugs, insulin and oral hypoglycemic agents.

Example of a 1500 Kcal diet

A diet must have a balanced supply of nutrients (energy needs and nutrients What foods to eat in a proper diet?), But the patient must also move and play sports

(The concept of lifestyle and the new food pyramid are news; the intake of fibers (diet and fiber) are necessary to encourage evacuation and fight colon cancer and to cure constipation.

Attention to anorexia!

She is a typical girl figure who thinks she is fat and thinks she has to lose weight. It's a voluntary reduction of food intake while the feeling of hunger is preserved for longer. Social activity (school and sports) is maintained. It's a psychological disease due to conflict with parents, rejection of one's own femininity, unreasonable fear of obesity. Sometimes it is associated with bulimia.

Amenorrhea (lack of the cycle) is constant, and the subject at the cost of losing weight gets used to vomit, to use strong laxatives, diuretics, etc. The doctor proceeds with investigations aimed at evaluating the gonadal functions, the level of estrogens and progestogens, FSH.

Therapy for anorexia

Progressive and planned re-training in agreement with the therapist who, initially, with health authority imposes stages in the re-establishment of the subject; in the most difficult cases the re-feeding with gastric tube can be indicated, with mixtures of amino acids that go above 2000 Kcal and must be hypoproteic, not exceeding 75 and 100 grams.

All this is associated with psychoanalytic or family and behavioral psychotherapy.
